This is a one time process that requires two acceptable forms of identification.
We use the accepted list of document found on the I-9 FORM published by the US Government.

Fill out the form below to get started. Don’t forget to upload photos to make your campaign shine.

Include photos in .jpeg or .png format (with a size less than 1200px by 900px) and under 8MB.
